This form includes submission fields for details about your presentation. The planning committee will work with you to confirm final learning objectives. A Professional Practice Gap is defined as the difference between health care processes or outcomes observed in practice, and those potentially achievable on the basis of current professional knowledge.
Monday, December 17, 2018 – Presentation Due
Presentations are required in advance to allow the planning committee to review each presentation, and to ensure content is appropriate for CME credit. The committee may also provide feedback to help you improve your presentation, and to make sure learning objectives outlined for the presentation were addressed.

If you plan to receive honorarium, or a travel reimbursement from UC Berkeley COEH Continuing Education, you must complete the following forms.

The W9 and Conflict of Interest form must be faxed together to (510) 664-7209. This is a secure fax line specifically designed to receive these types of forms, and to enter new vendors into the Berkeley payment system. Please contact firstname.lastname@example.org with any questions.

COEH Continuing Education provides honorarium as payment granted to an individual in recognition of participation at COEH Continuing Education events. The standard honorarium for speakers is $100/hour of instruction, assuming the speaker’s visa and tax status allows the acceptance of honorarium. COEH Continuing Education does not provide honorarium to University of California Berkeley employees, or to University of California employees at alternate campuses.
Course Materials & Deadlines
COEH has gone paperless. In this effort, we request presentations be submitted in advance. This allows us to share appropriate materials with the learners before the course, and to ensure the presentation works as anticipated on conference laptops, projectors, etc.
We typically save presentations as PDFs with 2 slides per page and the COEH Logo, and make these materials available to registered learners for download in advance. You are not required to share your presentation, and may also choose to submit a redacted version for distribution.
If you would like us to print worksheets or similar materials, please provide them no later than 2 weeks in advance of the course.
When submitting your headshot, please provide a photo that is at least 512px by 512px. This photo will be hosted on our registration website, next to your name and biography. We may also use this photo to help promote the course through publishing ‘Speaker Features,’ and highlighting your presentation. If you do not want your photo to be used to promote the course, you do not need to upload it to the Speaker Information Collection Form.
